Job Description:
TerraBella Senior Living is the proud operator of more than 20,

throughout the Carolinas, Virginia and Georgia. TerraBella
communities together account for more than 1,500 units and span a
full spectrum of senior living and care options, including Active
Independent Living, Assisted Living, Memory Care and available,
short-term Respite Care. -TerraBella Senior Living is hiring a
Resident Care Coordinator to join our community _TerraBella Little
Avenue_.The Resident Care Coordinator is responsible for the
coordination of resident care needs including, but not limited
todrafting the schedule, maintenance of required logs such as the
physician appointment log and the pharmacy log, filingand
organization of the clinical record, retrieval of signed physician
orders, etc. Additionally, the Resident CareCoordinator assists the
Director of Health & Wellness, as needed, to train new Care
Managers, and MedicationTechnicians. In the absence of the Director
of Health & Wellness, assumes the responsibility of supervision and
deliveryof resident care services in compliance with Terra Bella
expectations, policy and procedure and State regulations.
Responsibilities:

for review in the event that a Support Nursecannot complete the
audit.--- Assists with data collection, reporting processes as
requested.--- Performs a daily review of medication administration
process ensuring medications are passed timely andaccurately.
Intervenes as needed to provide support/ training during the med
pass.--- Performs as a preceptor to new team members ensuring each
new team member is in full compliance with allpolicies and
procedures relevant to their individual roles before completion of
orientation.--- Takes physician's orders following the medication
order process and maintains adequate records of ordertranscription
and verification.--- Ensures that all Residents Rights are
protected.--- Gives emergency treatment when required and notifies
the physician and family or responsible party of theemergency.
Completes resident, staff, and visitor incident and accident
reports.--- Notifies the Director of Health & Wellness of changes
in resident's condition, and any unusual or abnormalobservations.
(In the absence of the Director of Health & Wellness, notifies the
physician directly) Ensuresappropriate documentation is entered
into the resident's record and the 24-hour shift report relevant
tochanges in condition and physician communication.--- Schedules
appointments, and arranges for transportation of residents for
diagnostic, therapeutic, dental ormedical services. Ensures
compliance with the physician visit log process/procedure.---
Understands and implements the Fire Safety and Disaster Plan and
evacuation procedures of the communityduring drills and actual
emergencies.--- Performs all other duties as assigned by the
Director of Health & Wellness.--- Performs initial review of the
monthly Medication Administration Record verification to ensure
orders arecompliant.--- Assists the Director of Health & Wellness
to ensure compliance with all required logs, policies and
proceduresrelevant to medication administration.--- Ensures care
staff understands their role as it relates to dietary services
including how to serve, the requirementto pour water at every meal,
and how to ensure the right resident receives the right diet
prepared the right wayaccording to the posted diet board.---
Assists to serve residents during meal time, circulating through
both AL and Memory Care dining rooms.--- Performs building rounds
at the beginning and end of each shift, evaluating the care
provided and ensuringresident needs are met.--- May be required to
work overtime in an emergency situation.--- Sets a good example for
residents and other staff members by appearance, conduct and
interest in job.--- Takes weekend call in rotation with other
managers.--- Participates in Manager on Duty
