OPEN POSITION: Patient Care Coordinator Must Speak Spanish SCHEDULE: Full-Time Monday to Friday Business Hours Some Flexibility Offered COMPENSATION: $50,000 to $55,000 Base Salary, negotiable dependent upon experience Health/Dental/Vision Insurance Paid Time Off Package LOCATION: remote / work-from-home setting corporate office in Fort Lauderdale, Florida COMPANY PROFILE: This innovative, technology-forward medical practice is a fast-growing remote patient monitoring and virtual care provider that has proven success in positive outcomes for patients, while also lowering the cost of care. They are a 50-state provider network and have built a best-in-class technology platform to deliver excellent, relationship-driven care for patients with chronic conditions, in collaboration with health systems and health plans. Care is coordinated with the support of dedicated assistants, allowing providers to give their patients attentive and full care via 100% telemedicine. POSITION DESCRIPTION: The Patient Care Coordinator is responsible for liaising between internal and external providers, patients, and clinical staff, as well as establishing and maintaining relationships between the company and qualifying patients and their caregivers. Among other duties, the Patient Care Coordinator will: serve as a key touchpoint to patients assist in the coordination of care with patients--- PCPs and specialists triage phone messages from patients and other medical practices answer questions for patients and help them understand the process conduct follow-up and reminder phone calls to patients manage provider schedules and assist with coordination of patient scheduling manage the medical device ordering and replacement process help patients troubleshoot technology issues assist in the insurance verification process, including prior authorizations process patient information into the EHR and other software provide general administrative support to providers and management The practice uses Salesforce Health Cloud and DrChrono.
