($1,250 at 3 months of employment; $1,250 at 6 months of
employment; $2,500 at 12 months of employment) This Opportunity:
The primary responsibilities of the Assertive Community Treatment
Team Registered Nurse are to assist and support individuals who are
served in the development of skills, behaviors, responsibilities,
and supports needed to live, learn, work and be integrated into the
community with the greatest possible degree of self-determination
and independence. This person will provide nursing services
including administration and monitoring of medication, nursing
assessments and interventions. What You'll Do: • Deliver nursing
services, case management services, and supportive counseling to
the individuals served by the Assertive Community Treatment Team. •
Provide medication monitoring, medication administration,
medication education. • Review laboratory values and report to team
psychiatrist. • Evaluate physical health and make appropriate
referrals t. community physicians • Order and monitor medical
supplies. • Evaluate potential bloodborne pathogen exposure
incidents. • Link individuals with the Assertive Community
Treatment Team psychiatrist. • Facilitate training of independent
living skills, establish access to community services and agencies,
assist individuals in establishing support networks in the
community and assess individual needs on a daily basis. •
Coordinate services with the program, staff, inpatient facilities,
and detoxification facilities and establish linkage to residential
programs, self-help programs and community resources. • Review
treatment progress and monitor and provide continuous consultation
with other Assertive Community Treatment Team staff and community
agencies to ensure the consistent therapeutic management of the
individual’s needs. • Act as liaison with the program after hours
staff on a daily basis through exchange of participant information;
briefly detailing the presenting problem and after hours plans for
the individuals being served by the Assertive Community Treatment
Team. • Meet regularly with the other Assertive Community Treatment
Team members to formulate and disseminate information and be
available by telephone. • Complete an intake assessment including:
medical history, general physical condition, alcohol and drug
histories and current use, educational issues, family structure and
relationships, vocational history and goals, legal history and
issues, psychiatric history (including, onset, course, effect of
illness, treatment history, medication history risk behaviors). •
Ensure immediate and appropriate interventions are provided in
response to changes in mental status which may put individual at
risk. • Documents participant activities consistent with the
service records manual. Write an event note that reflects
participant activities, goals, interventions, and outcomes. •
Developing service plans and/or person-centered plans as
appropriate, and screens referrals in coordination with the other
Assertive Community Treatment Team members. • Meet regularly to
review the total caseload, problem-solve, develop plans, and
determine daily duty assignments. • Attend and actively participate
in meetings and training as required. Maintain certification in all
agency, state and federal training requirements. • Demonstrate
knowledge of and comply with all agency policies and procedures, as
well as service definitions related to specific program areas. •
Provide supervision to LPN level staff as needed/required. •
Driving and travel may be required. M0NC Education We're Looking

For: Drivers License (Valid) - USA, Registered Nurse (RN) - State
Board of Nursing Experience We're Looking For: Experience working
in ACTT Services | Not Required&xa;Experience working with
adults with mental illness | 1 Year | Required&xa;Psychiatric
Nursing Experience | 1 Year | Required&xa;Working knowledge of
psychiatric medications | Required Schedule: Monday-Friday
