Location: Remote Temporary opportunity Weekly Pay with direct deposit or pay card When you work through Team1Medical, you are eligible to enroll in dental, vision and medical insurance as well as 401K, direct deposit and our referral bonus program Job Description We are seeking a dedicated and compassionate Remote Community Health Worker to join a virtual care team delivering responsive, patient-centered telehealth services. This is a non-clinical role with a strong focus on supporting individuals managing substance use disorders, primarily through Medicaid-covered services . In this role, you’ll help coordinate care, facilitate telehealth appointments, and support client engagement in treatment plans. You’ll be a vital point of contact for patients and providers, contributing to a high-quality virtual healthcare experience. Responsibilities: Support patients in accessing virtual care services via Zoom and other telehealth platforms Help coordinate telehealth appointments and follow-up care plans Serve as a liaison between patients, providers, and care teams Provide education, reminders, and check-ins to promote engagement and treatment compliance Maintain accurate documentation and timely communication Collaborate with clinical teams to ensure seamless patient care delivery Qualifications and Requirements: Prior experience in community health, substance use support, case management, or patient coordination Comfortable working remotely and independently Excellent communication and organizational skills Familiarity with Medicaid and telehealth practices is a plus Tech-savvy – able to navigate Zoom and EMR systems with ease Must have reliable personal work equipment Flexible availability within the posted schedule Benefits and Perks: $20-$21/hr. If you are hired, your base salary will be determined based on factors such as individual skills, qualifications, experience, and geographic location. In addition, we also believe in the importance of pay equity and consider the internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is the full base salary range for the role. Hiring at the maximum of the range would not be typical in order to allow for future & continued salary growth.
