Responsibilities About the Role During visits that may last up to one hour, our providers assess and evaluate a member’s current health status, medical history, medication compliance, social environment, and other potential risks. This approach provides invaluable insights into a member’s overall health that are often missed during standard office visits. The Comprehensive Health Assessment (CHA) enhances care quality and allows us to address multiple care gaps in a single visit. Our providers: Conduct assessments for adults and geriatrics, including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-ups, and thorough documentation in accordance with our guidelines and protocols. Administer and order necessary point-of-care and lab-based tests. Collaborate with physicians, case managers, social workers, family members, key caregivers, and other medical personnel as needed. Work with Primary Care Physicians (PCPs) on patient education and follow-up as required. Provide services at the member’s residence, which may include their homes or skilled nursing facilities. Benefits of Joining Our Team Consistency – While assessments are conducted from Sunday to Saturday between 8 AM and 7 PM, our full-time providers generally work Monday to Friday from 8:00 AM to 5:30 PM. Competitive Pay – Enjoy an annual base salary ranging from $105,500 - $168,400 , determined by market analysis and experience. Incentive Program – Offers uncapped quarterly incentive opportunities. Along with thorough onboarding training, you will be assigned a preceptor, receive personalized feedback, and gain free access to online continuing education. Additional Licensure – If business needs arise, we will assist you in obtaining additional state licensure and credentialing in neighboring states or others you may wish to visit while conducting health assessments. Qualifications Skills & Experience That Stand Out Master’s Degree or equivalent experience along with successful completion of NP licensure. Current NP licensure in the state of practice, including prescription authority or the ability to obtain it as necessary. AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty. Current BLS, ACLS, or CPR certification. A minimum of 1 year of NP experience. Willingness to travel up to 80-90% of the time, including overnight travel and weekends across the assigned state and possibly neighboring states. A valid state driver's license and proof of adequate automobile insurance for the state of residence. Strong computer skills and familiarity with health/medical record software.
