Utilizing the nursing process, the Licensed Practical Nurse (LPN)
delivers patient care under the guidance of professionals licensed
by a State regulatory board to prescribe treatments and
medications, or under the supervision of a Registered Nurse (RN).
The LPN is accountable for the care provided to assigned patients,
coordinating efforts with various departments while ensuring
patient rights and confidentiality are upheld. All responsibilities
are carried out in alignment with our mission, values, and service

Positions: Part-Time and Full-Time LPN 12-Hour Shifts Role Summary
The LPN is responsible for delivering exceptional patient care
under the supervision of licensed professionals who can prescribe
medications and treatments or under the direction of an RN. This
position emphasizes safe, compassionate, and effective care through
the nursing process, maintaining accountability, and working
collaboratively with multidisciplinary teams. The LPN is committed
to upholding patient rights and confidentiality while performing
duties consistent with our mission, values, and service standards.
Main Responsibilities Assist RNs with the admission and discharge
processes. Conduct patient assessments and promptly communicate any
changes. Utilize safety tools (e.g., SBAR, bedside report) for
patient handoffs. Work with RNs to assess patient progress and
document interventions. Support patient and family education,
documenting learning needs. Delegate tasks to Unlicensed Assistive
Personnel (UAPs) and ensure care is within their competencies.
Complete necessary competencies and certifications, participating
in process improvement initiatives. Act as a preceptor for LPN
students or new hires as needed. Take on additional
responsibilities to support patient care and departmental
objectives. Qualifications Completion of an accredited practical
nursing program. Current LPN license in the state of practice.
Basic Life Support (BLS) certification within 30 days of hire.
LPN-IV certification upon hire or within the required timeframe.
Additional certifications may be necessary based on the unit (e.g.,
ACLS, PALS, PEARS). Physical Demands Capability to push, pull, and
lift up to 50 lbs regularly. Extended periods of standing and
walking during shifts. Frequent bending, kneeling, reaching,
twisting, and squatting. Experience Open to both new graduates and
