Our Nurse Practitioners are fundamental in crafting meaningful patient experiences, delivering personalized care aligned with our philosophy of dignity, comfort, and peace in lifes critical moments. Conduct comprehensive assessments of patient health needs and develop appropriate care plans Manage symptom control and prescribe medications as needed Provide direct patient care to hospice and palliative care patients Collaborate with physicians, nurses, and other healthcare professionals to optimize care plans Guide and support patients and their families through end-of-life care decisions Ensure consistent and accurate documentation of patient care and progression Participate in training and professional development activities Strong clinical skills with a focus on palliative care and symptom management Excellent communication and interpersonal skills Ability to make critical decisions in a high-pressure environment Comprehensive knowledge of healthcare regulations and hospice care policies Proficient in using medical software for patient management Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current licensure as a Nurse Practitioner in the state of practice Certification in Hospice and Palliative Care Nursing (CHPN) preferred
