PACE of the Southern Piedmont (POSP) is a not-for-profit, community-based organization committed to the wellness of seniors and is seeking a Home Care Lead Aide. HCA Lead Aide Home Care Lead Job Summary: Performs patient care activities in the community and at the Adult Day Health Center. Will assist with scheduling and assignments. Will assist with training and mentoring of CNA/PCA staff. Covers absences and Start of Care Services. Will update team members. Reports directly to the Home Care Agency Supervisor.Education: High school diploma or equivalent. Currently listed as NA1 or NA2 with NC Department of Health and Human Services or NC Board of Nursing with no substantiated findings of neglect, abuse, or misappropriation of resident's property in a nursing home.Experience: 1-2 years' experience as a nursing tech/nursing aide, or EMT or corpsman. At least 1 years' experience working with the frail elderly population.Licensure, Registry or Certification Required: Valid NC driver's license and vehicle. Certificate of completion from a nursing tech/aide program.Special Training: Good driving record; able to communicate effectively both verbally and in written form; able to position and turn participants; knowledge of and/or care of adults with chronic health care conditions. Only act within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by Senior Total Life Care, Inc. and approved by CMS before working independently.Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact Key ResponsibilitiesProvides personal care (bathing, dressing, feeding, toileting, transfers, and incontinent care) to participants of PACE of the Southern Piedmont in the Home and in the Adult Day Health Center.Responsible to report changes to Home Care Agency Supervisor.Supports PACE of the Southern Piedmont's mission to encourage and support the quality of life of seniors wishing to continue living in the community; its vision to be the preferred provider of individualized care for seniors in the community; and its values of respect, integrity, accountability, compatible goals, and compassionate care.Documents services provided and observations on appropriate forms.Participates in the on-call process.Provides routine rounding times for CNA members on the team.Will be flexible in hours worked to accommodate the needs of the participants in the homes.
